29-year-old Bryan Whelan and 36-year-old TJ Herlihy were carrying out remedial works on Thomond Bridge in Limerick on Saturday when the cage they were harnessed to broke free from a crane, plunging into the River Shannon.

Following a massive rescue operation, the two men were pronounced dead. A third man, 36-year-old Paul Murphy from Ballysteen in Limerick, managed to free himself and escaped with only minor injuries.

An investigation, led by the Health and safety Authority has been launched and investigators are now gathering information for their report with the assistance of Gardaí.

Under current practice, reports are issued to the HSA's internal legal department and the office of the DPP but are not made public, though families are kept informed of the progress of investigations.

The HSA have told The Irish Examiner that there is no time scale for the investigation.
