Healthcare agencies across the Midwest have developed a new five-year strategy for palliative care in Clare and surrounding counties.
The plan, jointly developed by Milford Care Centre, HSE Mid West Community Healthcare and the UL Hospitals Group, will function as a roadmap for end of life care over the coming years.
Specific targets contained within the strategy include a reduction in unnecessary hospital admissions and improved access to palliative care throughout the region.
