A number of healthcare staff involved in the care of Savita Halappanavar have been "disciplined".
Savita died at University Hospital Galway in October 2012 – a week after she was admitted to the hospital when she was 17 weeks' pregnant and miscarrying.
Inquiries into her death identified "multiple shortcomings" in her care, and the group responsible for the hospital started a review process to examine all staff members involved.
Of the 30 staff whose actions were considered – 21 were found to have "no case to answer" – and weren't disciplined.
However, some were issued written warnings – which outlined "necessary improvements" they have to make – and a timescale to do it.
While other staff were told to undertake "pre-procedural" informal counselling – which involves telling staff what aspects of their work need to be bettered – and what supports can help achieve that goal.
The West-NorthWest Hospital group says the disciplinary process against most of the 9 staff is now complete.
